Dental plan offer terms and conditions

*This offer is only available to new and existing patients who sign up to a new dental payment plan with participating practices between <insert date> and <insert date> inclusive. No joining fee will be charged where a patient signs up during the offer period. Patients must ages 10+ to receive this offer. Eligible patients will receive one Philips Sonicare 3100 electric toothbrush free of charge, subject to availability and these terms. Patients will receive an email from their practice with a code to redeem their free toothbrush on Philips website after the first successful monthly payment has been taken under the dental payment plan. Philips will be responsible for dispatch and delivery of the electric toothbrush; any issues with delivery will need to be referred to Philips consumer care team.

This offer is only valid with participating clinicians and specific dental plan T&Cs apply and vary by provider.

Patients must be dentally fit to proceed with certain dental payment plans. Whether a patient is deemed dentally fit for treatment will be at the clinician’s absolute discretion. Additional treatment required to make a patient dentally fit and suitable for will be charged at an additional cost.

Appointments are subject to availability. This offer is non-exchangeable, non-transferable, and is not redeemable for cash or other treatments. In the event that participating practices is not able to facilitate an appointment scheduled in relation to this offer, participating practices will use best efforts to rearrange the appointment.

This offer cannot be used in conjunction with any other offer. Participating practices will exercise reasonable care to ensure that the treatment provided is safe and effective. Please note that the participating practice accepts no liability on behalf of the treating clinician and acts strictly as a liaison between the patient and the treating clinician only.

Nothing in these terms and conditions excludes or limits liability for personal injury resulting from participating practices negligence or for fraudulent misrepresentation, or for any other liability which cannot be limited by law. Participating practices can vary or replace these conditions from time to time, at its sole discretion and reserves the right to withdraw this offer at any time. If any provision or part-provision of these terms and conditions is or becomes invalid, illegal or unenforceable, it shall be deemed deleted, but that shall not affect the validity and enforceability of the rest of the terms and conditions.
